What actually differs?

Ozempic and Wegovy are the same molecule (semaglutide) at different licensed doses for different indications. Ozempic caps at 2.0 mg for diabetes; Wegovy goes to 2.4 mg for weight management. The pens differ; the peptide does not.

Mounjaro is a different molecule entirely — tirzepatide, a dual GLP-1/GIP agonist manufactured by Lilly. In SURPASS-2 it out-performed semaglutide on weight loss and glycaemic control at matched cadence.

Same drug, different pens: Ozempic vs Wegovy

Novo Nordisk licensed semaglutide twice under two brands because the maximum weekly dose needed for chronic weight management (2.4 mg) exceeded the maximum weekly dose approved for type 2 diabetes (2.0 mg). Rather than expand the Ozempic label, Novo pursued a separate indication — resulting in Wegovy. Chemically identical. Marketed and priced separately.

Rybelsus: oral semaglutide

Rybelsus is the third semaglutide brand — an oral tablet using SNAC absorption enhancer, licensed for type 2 diabetes. Bioavailability is low (~1%), so tablet doses are much larger than injection equivalents. Included here because searchers often bundle it with Ozempic and Wegovy.

Mounjaro and Zepbound: same compound, two brands

Lilly followed the Novo playbook: Mounjaro is tirzepatide licensed for type 2 diabetes; Zepbound is the same tirzepatide licensed for chronic weight management. Identical molecule, different indication and label.
